Privacy and Data Protection:
The Director of Governance and Risks, Hebert Luiz Gomide Filho, is responsible for processing Personal Data at Caixa Seguridade. Important: in order to validate the ownership and legitimacy of the request, it is necessary to collect personal data and documents to assist in the authentication process. Therefore, when sending the request, via email or form, the following must be made available:
Right holder: Provide personal data and send, by photo or scanned, the following proof:
- – Identification document with photo (Driver´s license, ID, functional identity or passport).
- – Proof of residence (water, electricity, internet or telephone bill). If the proof is not in the name of the right holder, the available document must be sent and the reason justified in the request.
Third party rights: To exercise the right on behalf of another person, the personal data of the applicant and the right holder must be provided, as well as sent, by photo or scanned, the following proof:
- – Identification document with photo of the applicant and the right holder (Driver´s license, ID, functional identity or passport).
- – Proof of residence of the applicant and the right holder (water, electricity, internet or telephone bill). If some of the proof is not nominal, the available document must be sent and the reason justified in the request.
- – Power of attorney or other official document proving your guardianship/representation.
The data sent will be stored exclusively for legal and regulatory purposes, proving that the request was made and processed, thus aligning with the provisions of the General Data Protection Law – LGPD (law no. 13,709/18).
